1045 Characteristics and outcomes of women with MEWS (Maternal Early Warning Signs) in a community hospital

MEWS are a routine feature of many obstetrical safety programs. However, there limited published data on their impact. Our objectives were to assess the frequency, severity, and outcomes women with trigger events at community hospital. We performed retrospective cohort study all for pregnant post partum occurring Level 2 hospital from 10/1/2019-8/31/2020. criteria followed definitions proposed by California Collaborative: systolic blood pressure less than 90 or greater 160 mmHg, heart rate 50 120 beats per minute, respiratory 10 30 breaths oxygen saturation 95%, urine output 35 milliliters hour hours. Maternal newborn charts reviewed clinical details. Timing (antepartum, intrapartum, postpartum), primarily etiology (hypertension, infection, hemorrhage), analyzed. Out 1,582 deliveries, 41 patients (2.6%) had MEWS. these, 24 (59%) multiple triggers; 74% occurred in postpartum period 69% who underwent cesarean delivery. Hypertension accounted 87% 81% resulted intervention, whether it was medication administration (75%), Intensive Care Unit admission (1%), consultation specialty service (9%), maternal transfer higher level care (2%). After event, no required rapid response team (RRT) call. In hospital, triggers 2.6% hospitalizations; most timing, following delivery, related HTN, almost always change care.
